The management of pregnancy and childbirth in women after conservative myomectomy is complex, multifaceted, and highly relevant to modern reproductive medicine. It requires further in-depth scientific research aimed at developing objective and precise criteria for assessing the integrity and functional adequacy of the uterine scar.
Aim – To analyze data from contemporary scientific literature on reproductive outcomes after conservative myomectomy and to compare published findings with our own clinical observations.
Materials and Methods. A review of current literature sources addressing the management of pregnancy and childbirth after conservative myomectomy was conducted. Key challenges and diagnostic issues faced by researchers and clinicians are outlined.
Results. Issues related to the surgical techniques of myomectomy and myometrial repair, as well as the specifics of delivery management in patients with a history of conservative myomectomy, were examined. Features of postoperative complications and recurrence were identified.
Conclusions. Optimization of existing surgical myomectomy techniques to minimize risks for future pregnancies, along with improvement of strategies for pregnancy and childbirth management in this group of women. An individualized approach to each patient, taking into account the full range of clinical data, careful monitoring throughout pregnancy, and effective communication among obstetricians-gynecologists, surgeons, and patients themselves, are key elements in achieving the best possible obstetric and perinatal outcomes for both mother and child. The development of international consensus statements and clinical protocols may significantly improve the quality of care for these women.
