Research objective. Evaluation of echographic features of uterine fibroids in young women.
Materials and methods. To achieve the stated objective, this study prospectively and retrospectively included examinations of 101 patients with uterine fibroids (ranging from 1 to 9 cm in diameter). The examination of all patients began with a detailed medical history collection using specialized charts, taking into account risk factors for the development of the disease. In the course of the examination, a complex of diagnostic techniques was used, such as: medical history data, clinical and gynecological examination, histological examination of endometrial scrapings, uterine cavity aspirate, colposcopy, ultrasound diagnostics, endoscopy (hysteroscopy), laparoscopy, immunohistochemistry, and morphological study of macro-specimens removed during myomectomy. Biochemical, hemostasiological, and other laboratory parameters were also taken into account.
Results. Proliferating uterine fibroids in young women was characterized by: a heterogeneous, "mottled" structure of myomatous nodules; neovascularization; blood flow with low vascular resistance, sinusoidal vessels with high velocity and varied directions; blood flow with pronounced color signal brightness and a "mosaic" pattern of mapping, high density of color signals; resistance index in the myometrium - 0.53±0.09, in myomatous nodules - 0.34±0.06; Vmax of arterial blood flow - 28.9 cm/sec; numerous veins in different parts of the myomatous nodule with a Vmax of 12.8 cm/sec.
Conclusions. Thus, when examining patients with uterine myoma, echographic examination should be performed alongside color Doppler mapping data. Using this additional technique, it is possible to record the location and number of vessels in the myomatous nodule, determine the velocity and intensity of intratumoral blood flow, and characterize the morphological forms of uterine fibroids. The obtained results must be taken into account when diagnosing uterine fibroids in young women.
