The aim is to study the features of the fetoplacental complex (FPC) in women with a history of infertility after the use of assisted reproductive technologies (ART) to develop indications for programmed delivery in them.
Materials and methods. 60 women with a history of infertility and who became pregnant as a result of the use of ART were examined. Group I - 30 pregnant women after ART with a history of infertility, who were delivered abdominally and carried out before and during pregnancy according to the generally accepted method, Group II - 30 pregnant women after ART with a history of infertility, for whom we programmed pregnancy and childbirth, and during pregnancy we carried out a complex of therapeutic and preventive measures aimed at preventing obstetric and perinatal complications. Control group (CG) - 30 pregnant women who were delivered through the natural birth canal without obstetric and somatic pathology. To assess peripheral vascular resistance, the systolic-diastolic ratio (S/D), pulsatility index (PI) and resistance index (RI) were determined; hormonal function of the FPC was determined in blood serum by radioimmunological method, the level of hormones: estriol, estradiol, progesterone, prolactin, chorionic gonadotropin and placental lactogen.
Results. The results of Dopplerometry of the umbilical artery indicate that in group I at 32-33 weeks there is a significant decrease in the S/D ratio to 2.09±0.05; PI to 0.71±0.03 and IR to 0.45±0.02; in the fetal aorta there is a significant decrease in the S/D ratio to 4.87±0.21, as well as PI to 1.54±0.03 and IR to 0.75±0.04; on the eve of delivery, a significant decrease in the S/D ratio to 4.53±0.15.01), PI to 1.350.04, IR to 0.68±0.03, and starting from 28-29 weeks, pregnant women of group I had a significant decrease in estriol - to 23.45±2.06 nmol/l and estradiol - to 37.83±2.05 nmol/l.
Conclusions. The state of blood circulation in the FPK in pregnant women with a history of infertility, after ART, is characterized by a significant decrease in the S/D ratio, PI and IR in the umbilical artery, which persists until delivery; and hormonal disorders are characterized by a significant decrease in the level of progesterone, estriol and estradiol.
The study was performed in accordance with the principles of the Declaration of Helsinki. The study protocol was approved by the Local Ethics Committee of the institution specified in the work. Informed consent was obtained from women for the study.
